We hereby inform you that you will be submitting your own and/or your foreign worker’s Personal Data to e-MEDIK for the purpose of conducting medical examinations for foreign workers seeking employment in Malaysia. Kindly take note of the following important points:

Your own and/or your foreign worker’s Personal Data requested in the Foreign Worker Medical Examination Registration Form is necessary to facilitate the medical examination process.

The information you provide to e-MEDIK constitutes Personal Data as defined under the Personal Data Protection Act (PDPA) 2010.

By furnishing this Personal Data, you hereby consent to e-MEDIK to:

  1. Collect, store and process your own and/or your foreign worker’s Personal Data.
  2. Disclose such Personal Data to relevant government agencies or authorised third parties where required by law or for legitimate legal purposes.

The Personal Data required includes, but is not limited to, names, identification details, and contact information obtained through online platforms or registration forms managed by e-MEDIK.

In addition to the information you directly provide, e-MEDIK may also source and process Personal Data from the following:

  • Passport, visa, calling visa, or permits submitted to e-MEDIK
  • Participation in surveys, contests, promotions, or programmes organised by e-MEDIK
  • Any commencement of business dealings or communication with us
  • Third parties connected to you (e.g. financial institutions or credit reference bodies)
  • Government or regulatory bodies with authority to release such information (e.g. Immigration Department, Ministry of Health)
  • Publicly available sources, or any sources for which you have granted consent
  • Any other sources permitted by law

e-MEDIK will ensure that your Personal Data and that of your foreign worker are safeguarded in accordance with the PDPA 2010 and all other applicable Malaysian laws.

You have the right to request access to your Personal Data, as well as to request corrections to any inaccurate or incomplete information supplied to e-MEDIK.

Your Personal Data will only be disclosed to third parties under the following circumstances:

  • To the Government of Malaysia; and
  • To our subsidiaries, affiliates, representative offices, or joint-venture partners, whether located within Malaysia or abroad.

You may also choose to limit the processing of your Personal Data by submitting a written request to e-MEDIK, specifying the type of restriction and the reason for such a request.

Providing your Personal Data to e-MEDIK is voluntary. However, failure to supply complete and accurate information may result in our inability to process or conduct the required medical examination.
